According to Medicare, this facility maintains an overall rating of one out of five stars, including two out of five stars concerning health inspections, one out of five stars for staffing issues and two out of five stars for quality measures.

  • Failure to Protect Every Resident from All Forms of Abuse, Physical Punishment, and Being Separated from Others – citation #F223 date May 1, 2017
  • The state investigators determined that “the facility failed to ensure each resident was free from abuse, neglect, misappropriation of resident property, and exploitation.” The investigation involved one resident “reviewed for abuse and neglect.”

    Specifically, a Licensed Vocational Nurse failed to assess the resident “on the significance of the resident’s pain and having the resident wait over an hour for pain medication.” The documentation shows that the Licensed Vocational Nurse was “loud and shook her head in [the resident’s] face, told the resident that the doctor should order pain medication differently and she would have to wait for the medication.”

    The Licensed Vocational Nurse “also told the resident she was working two halls.” These failures placed the resident “at increased risk for unrelieved pain-and-suffering, and the forty-six residents on pain management at risk for managed pain control and the fifty-one residents on Hall 100 and 300 where [the Licensed Vocational Nurse] worked for abuse and unmet needs.”

    The resident’s MDS (Minimum Data Set) Assessment reflected that the resident “was cognitively alert and oriented and able to make decisions and relate her needs without difficulty.” The resident’s Pain Assessment Scale reflected that the resident “was a high risk of pain at a level of twelve. [Any] level greater than ten was a high risk for pain.”

    The resident’s Care Plan reflected “she was at risk for pain. The goal was to be able to verbalize pain. The interventions were to monitor for signs and symptoms of pain, follow the pain scale, monitor and record the pain, observe for non-verbal pain, moaning, allow the resident to verbalize feelings of pain or discomfort.”

    The state surveyors observed and interviewed the resident on the morning of May 1, 2017. The resident stated that she “had her call light on and she had it on for a while, and she needed to go to the bathroom.” The resident “revealed she had a problem getting her pain medication on the night shift.”

    The resident revealed that on one night (April 23, 2014), “she was suffering from her leg, and she could not get her pain pill. The nurse came in and asked what she wanted, and when the resident asked for her pain medication, the nurse said she would have to wait.”

    When “the resident inquired further, the nurse got in her face and said she would have to wait, was loud, shook her head at the resident and said you are not the only one who gets medicine at 11:00 PM.”
