According to Medicare, this facility maintains an overall rating of one out of five stars, including two out of five stars concerning health inspections, one out of five stars for staffing issues, and three out of five stars for quality measures.

  • Failure to Protect Every Resident from All Forms of Abuse Including Physical and Sexual Assault, Physical Punishment, and Neglect by Anybody – citation #F600 date May 24, 2019
  • The state investigators determined that the nursing home “failed to protect [one resident] from resident-to-resident verbal abuse that happened during the night shift (11:00 PM to 7:00 AM shift). This deficient practice resulted in [the resident] feeling threatened, nervous, and anxious, and spending the night in the hallway.”

    The victim’s medical records indicated that the resident “was moderately impaired in cognitive skills and daily decision-making. The MDS Assessment indicated that the resident requires extensive assistance in walking in her room and the corridor and requires limited assistance in locomotion on-and-off the unit. This resident used a wheelchair from mobility.”

    A review of the verbally assaulting resident’s MDS (Minimum Data Set) Assessment revealed that the resident “was alert and able to make her needs known, requires supervision while walking in her room and the corridor with locomotion (how/when the resident moves between locations) on-and-off the unit.” The resident “used a walker for mobility.”

    The Police Department Crime Report dated February 1, 2019, indicated that two police officers “responded to a call from the facility” in the early hours. The aggressive resident had “made threats to kill and beat her roommate, [causing the roommate] to be in fear for her life.” The roommate stated that at 10:15 PM (on January 31, 2019), [the verbally aggressive resident] went to the shared room with [the roommate].”

    The roommate stated that the aggressive resident “made it clear to [the victim that she] hated people and did not want a roommate.” The victim stated that the aggressive resident fell asleep during that time, so she [the roommate] got up to see if the nurses received medication from the pharmacy.” The victim stated that is when the aggressive resident “had a change in behavior.”

    The police report indicated that the victim stated that she was threatened by the verbally aggressive resident while “grabbing the sliding curtain open and saying to her that she [the aggressive resident] would kill her [the victim] and she will use the stick that was by the patio door to beat her.”

    The victim stated that “after this incident, she texted the facility’s Marketing Director.” During the conversation with the police officer, the victim became “emotionally upset, she began crying, and was trembling in fear.”
