The federal government carries out annual inspections of skilled nursing facilities pursuant to its role as the overseer of the Medicare and Medicaid programs. The government's reimbursements to the nursing homes do not come without any strings attached. Instead, there must be compliance with federal regulations in order to ensure that these facilities will continue to receive federal funding in the future. The inspection reports are made public and you have the ability to know what issues are occurring at the facility where your loved one resides. 

Although the Lakeview Health Care & Rehabilitation Center has not received any fines from the federal government, the recent inspection reports have shown a negative trend in the quality of care administered to residents of the facility. The most recent inspection report, issued in March 2018 resulted in 13 health citations against the facility, which is well above the national average. 

In addition, the quality of care metrics show that this nursing home has a rate of hospitalization that is much higher than the national average. The Lakeview Health Care & Rehabilitation Center has 3.9 resident hospitalizations per every 1000 resident days. This is more than double the national average of 1.7. Medicare recently instituted a program where it can dock nursing homes up to two percent of their reimbursements for a high rate of hospitalization and this number puts the Lakeview Health Care & Rehabilitation Center in position to receive a penalty. 

There was an incident in the latest inspection report that involved an allegation of verbal abuse against a CNA at the facility. The nursing assistant was reported to have cursed at a resident when providing care. Another CNA stated that they heard the incident and corroborated the allegation. The CNA at issue claimed that the resident was being physically abusive. However, the home's policies clearly prohibit verbal abuse, even if the resident was being violent. The CNA was suspended during the course of the investigation and was terminated from employment at the nursing home. 

In another instance documented in the evaluation report, staff refused to give a resident a certain medication. Specifically, a nurse practitioner ordered a certain prescription, but the nursing staff did not give the medication to the resident. When asked, the Director of Nursing stated that they did not agree with the prescription so they did not give it to the resident. 

In another incident, a resident had engaged in behavior that indicated attempts to harm themselves. The Director of Nursing did not notify the resident's guardian or their psychiatrist about the behavior. In other instances, the staff did not notify the resident's physician of certain incidents. For example, the resident jumped out of a window and injured their leg, but staff did not notify the physician. 

In addition, there have been five complaints against this facility that have resulted in a citation in the past three years. In one instance, a resident overdosed on medication that they removed from a medication cart. The tablets that the resident took contained Hydrocodone and Acetaminophen. The resident ingested 31 tablets that they accessed.
