The state investigators determined that the nursing home “failed to ensure the nursing staff reported an allegation of abuse for investigation for [one resident].” The surveyors attempted to interview the resident who only turned his head and closed his eyes. A few minutes later, “he opened his eyes and seemed to look around the room but did not respond to any questions.” He was offered several items but did not respond to any offer.
A review of a Progress Note written on October 15, 2018, signed by a Licensed Practical Nurse (LPN) indicated that he was requested to come to a resident’s room. Upon entering the room, the LPN indicated that the resident had told him that “someone, specifically a male, has hit him.”
The LPN stated that this is not the first time the resident made an accusation. He “could not remember any name of who hit him, he began naming the sequence of names.” After this, the LPN inspected the resident’s body, including the “cheeks, head, neck, forehead, and orbitals for signs and symptoms of abuse.”
The Licensed Practical Nurse stated that after the assessment, it was noted that “all areas at this time of the assessment are free of signs of bruising, bleeding, or injury. The resident is alert to self. Not alert to time, situation, or place.”
The resident’s sister had noted that she had reported the comment that her brother had made to an assigned floor nurse and stated that “she had no evidence of any abuse, but her brother made the statement, so she felt she needed to report the information.” The sister indicated that “each time she comes to visit, which is every week, she finds minor issues and reports them to the nurses. She stated it is possible someone did hit him, but she does not have any evidence or further information.”
