According to Medicare, this facility maintains an overall rating of one out of five stars, including one from five stars for health inspections, two out of five stars for staffing issues, and three out of five stars for quality measures.

  • Failure to Protect Every Resident from All Forms of Abuse Including Physical and Sexual Assault, Physical Punishment, and Neglect by Anybody – citation #F600 date October 19, 2018
  • The state investigators determined that the nursing home “failed to implement its policy to protect [one resident] from being sexually abused by [another resident], by not monitoring both residents.” Documentation shows that on July 21, 2018, at 1:10 PM, one resident “sexually assaulted [another resident].” This “deficient practice resulted in [the sexually assaulted resident being] transferred to a General Acute Care Hospital.”

    The hospital report indicates that the resident “sustained a bruise to the right shoulder, right thigh, abrasions (area damaged by scraping or wearing away) to bilateral breast and nipples, bruises (skin discoloration) to the left side of the back, and a large open laceration (cut) to the posterior fourchette (small fold of the membrane connecting the labia minora in the posterior part of the vulva) of the vagina.”

    The facility’s Final Investigation related to the resident being abused was dated July 24, 2018. The document states that a facility laundry staff member observed the resident “on top of the female resident.” The laundry staff member stated that the male resident sexually assaulted the female resident through “sexual intercourse and penetration without her consent.”

    Documentation shows that the sexually aggressive male resident was “moderately impaired with poor decision-making and required cues/supervision for daily decision-making.” The Post-Event Assessment Form from the same day indicated that the resident was seen by the laundry staff member in the female resident’s room lying on top of the female resident. It also indicated that the female resident “was clothed,” and the male resident was “clothed and getting off [the female resident] and into his wheelchair.” The form indicated that the male resident “put his fingers to his lips in an ‘shhh’ manner and told the witness don’t say anything.”

    The report indicates that the laundry staff member had observed blood on the female resident’s clothing, the bed, and the resident’s hand. The document shows that when the resident was interviewed and asked about the incident, the male resident stated, “I was only kissing her.” The report indicated, “the blood was observed on [the male resident’s] left hand by the nursing staff.”
