  • Failure to Develop Policies That Prevent Mistreatment, Neglect or Abuse of Residents – citation date March 3, 2017
  • The nursing home “failed to conduct thorough investigations and the possible causes of injury of unknown origin” involving three investigations. In one incident, the surveyors reviewed a resident’s safety report that revealed that during morning care, the nursing staff noted bruising on the resident’s “lateral and medial aspects of the right foot. The resident denied pain and stated, ‘I do not know how it happened.’ The report included there were no definitive incidents identified that may have caused the injury. The section of the report that included the question was further investigation required was blank.”

    The survey team met with the facility Administrator and Director of Nursing after identifying a violation. The Director of Nursing stated that “no further investigation was conducted after the incident and there was no updated Care Plan regarding the incident.”

    A second incident involving a lack of follow-up for an injury of unknown origin was identified by the surveyor when a Certified Nursing Assistant (CNA) “notified the nurse the resident had purple discoloration on the right upper arm. The report concluded that there was no definitive origin for the injury identified.” The Administrator and Director of Nursing stated that “no further investigation was conducted after the incident and the Care Plan was not updated.”

  • Failure to Provide and Implement an Infection Protection and Control Program – citation date June 15, 2018
  • According to investigators, the facility “failed to follow appropriate handwashing practices to prevent the spread of infection during the wound treatment observation.” The surveyors interviewed a Registered Nurse (RN) who also served as the Unit Manager who identified the resident had “a deep tissue injury to the right heel.”

    The survey team observed a Licensed Practical Nurse (LPN) performing “a wound treatment to the resident sacrum and the right heel” and hand washing without using suds before sanitizing the side table.

    After sanitizing the side table in performing handwashing for sixteen seconds, the LPN never use suds “during hand scrubbing. The surveyor asked the LPN if there was soap in the dispenser and if it was appropriate to perform hand washing under the running water. The LPN informed the surveyor that there was not enough soap in the dispenser in stated that ‘owe no, I should not have washed my hands under running water.’”

    During an interview with the Vice President of Nursing and the Assistant Director of Nursing, it was indicated that “the LPN was nervous and not aware that she performed hand washing in less than twenty seconds. The Vice President of Nursing also stated that according to the LPN, there was not enough soap in the dispenser and it was only a little bit of so.” This deficiency increased the potential of the spread of infection in the facility.
