According to Medicare, this facility maintains an overall rating of one out of five stars, including one out of five stars concerning health inspections, two out of five stars for staffing issues and one out of five stars for quality measures.

  • Failure to Provide Every Resident an Environment Free of Accident Hazards and Provide Adequate Supervision to Prevent Avoidable Accidents – citation #F689 date June 7, 2018
  • The state investigators determined that the nursing home “failed to investigate falls, provide a safe transfer to prevent a fall, and Care Plan a post-fall targeted intervention for [one resident] reviewed for falls.” The investigation involved a moderately, cognitively impaired resident who “requires extensive physical staff assistance with transfers.”

    A review of the resident’s Fall Risk Assessment documented in March and May 2018, indicates that the resident “is at high risk for falls.” A report indicates that two Certified Nursing Assistants (CNAs) used a mechanical lift on the resident to transfer her. While the resident was in Medicare, they “proceeded to slide out of the mechanical lift stand sling and sit down.” The CNA stated “likely, the chair was in place. I and the other CNA left the room. Not even ten minutes later, I heard [the resident’s] alarm.”

    The CNA stated they entered the resident’s room to find the resident “in between the bathroom door with her pants at her ankles and her incontinence brief ripped.”

    In a separate summary statement of deficiencies dated June 23, 2017, the facility “failed to ensure a safe transfer of [a resident] while using a mechanical lift.” This investigation involved a moderately, cognitively impaired resident who “was totally dependent and needing two-plus persons physical assist during transfers.” The documentation shows that the resident has “impairment on both upper and lower extremities bilaterally.”

    The resident’s report and SBAR (Situation, Background, Assessment, Recommendation) indicated that the resident had been transferred by unidentified students in the Certified Nursing Assistant program. The “students were using a mechanical lift and [the resident] slid out of the sling and was assisted to the floor by the students.”

    The documentation indicates that the “students did not ask the facility CNAs for help in the transfer of [the resident], nor was the student’s instructor present during the transfer.” The documentation indicates that the students were re-educated that “they need staff supervision or instructor supervision for mechanical lift transfers.”
