  • Failure to Develop, Implement and Enforce Policies That Prevent Mistreatment, Neglect or Abuse of Residents – citation #F226 date October 19, 2017
  • The state investigators determined that the nursing home “failed to follow their Abuse Policy when [staff] was unaware of all types of reportable abuse per facility policy [and] staff was unaware of reporting abuse with serious bodily injury within two hours. These failures place residents at risk of abuse from potential staff, delaying care, and delay of an investigation.”

    An interview with an Activity Director regarding abuse policies and procedures revealed that “she was unable to identify all reportable types of abuse per the facility policy.” The Aide “was also unable to identify when to report abuse with serious bodily injury.”

    This failure to understand how to report and handle abuse cases also involved a housekeeper who could only list two (physical and verbal) of the seven types of abuse that need to be reported to the Abuse Coordinator.

  • Failure to Ensure the Nursing Home Area Remained Free of Accident Hazards and Risks and Provides Supervision to Prevent Avoidable Accidents – citation #F323 date October 19, 2017
  • The “facility failed to provide a safe environment for [one resident, who had ‘only one side of his bed [covered] with the floor mattress.’” The resident had a Physician’s orders to have a bilateral mattress on the side of the bed. This “failure has the potential for placing the resident at risk for serious injury should he fall on the side of the bed without the floor mattress.”

    A review of the resident’s records indicates that the resident has progressive mental disorientation “that can occur in the middle of old age, due to generalized degeneration of the brain and a history of falling.” The state surveyors and a Licensed Vocational Nurse performed an initial tour of the facility and observed the resident “in bed with the floor mattress on the right side. On the left side of the bed, there was no floor mattress observed.”

    The Licensed Vocational Nurse replied that “she was unsure why there was only one mattress and acknowledged there should be a mattress on both sides of the bed.”
