According to Medicare, this facility maintains an overall rating of one out of five stars, including one out of five stars concerning health inspections, two out of five stars for staffing issues, and four out of five stars for quality measures.

  • Failure to Provide Different Approaches before Using a Handrail – citation #F700 date February 5, 2018
  • The state investigators determined that the nursing home “failed to ensure the consent form signed by the sponsor detailed the potential risks associated with the use of side rails.” The nursing home also failed to ensure that “the side rail assessment addressed prior alternatives attempted before installation and the side rail assessment included an assessment for the risk of entrapment.”

    The investigators reviewed the facility’s policy titled: Proper Use of Side Rails that specifies its purpose “is to prevent resident injury and serve as an enabler for the resident.” The survey team requested a copy of the resident’s consent for the use of side rails that “was signed by the sponsor prior to the resident’s initial admission. This form included the following disclaimer: although we will do our best to prevent problems, based on the physical, mental, and medical conditions, things that can happen to them while they are here include – at risk for falls related to past history. Will utilize side rails and a low bed and any other intervention necessary.”

    However, the surveyors determined that “the form failed to document the risks imposed by the use of side rails.” The Admission Agreement signed by the sponsor approved the use of bed rails. However, there was “no mention made of the potential risks involved.”

    The investigators observed the resident on February 2, 2018 at 8:16 AM and again on February 5, 2018 at 9:15 AM. On both of those occasions, the resident “was observed in a low bed with four side rails up.”

    Just after the second observation, the surveyors asked the Certified Nursing Assistant (CNA) providing the resident care “why four side rails were in use on the resident’s bed.” The CNA explained that the rails were there because the resident “had a fall resulting in a right shoulder injury.” The CNA explained that “the resident could move his/her legs, but very little since the November 17, 2017 fall which resulted in a fractured (right) shoulder.” The CNA was not sure why the “lower side rails were in place.”
