  • The state investigators determined that the nursing home “failed to ensure that each resident received adequate supervision and assistance devices to prevent accidents for [one resident] reviewed for accidents and supervision. The facility failed to supervise [a resident] who had exit-seeking behavior.”

    As a result of the deficient practice, the resident “eloped [wandered away] from the facility on June 11, 2017, at 4:30 AM. He was found on a busy highway by the police department and returned [that morning] at 5:25 AM.” The state investigators identified in Immediate Jeopardy because it could have caused harm or death to the resident.

    This “failure placed seven residents identified as being at risk for elopement and utilizing a Wander Guard system at risk for development, serious injuries, or death.” A review of the resident’s Care Plan and Elopement/Wandering Evaluation indicated that the resident “had an exit seeking/wandering behavior and had been categorized as high risk with the score of sixteen.”

    The documentation initiated on June 9, 2017, indicated that the resident “was an elopement risk/wander related to impaired safety awareness [and] wanders aimlessly, history of exit-seeking at home. No goals were identified.” A review of the Provider Report dated June 11, 2017, revealed that at 3:00 PM, a Licensed Vocational Nurse noticed the resident “wandering on the hallway and redirected him back to his room.”

    By 4:28 AM, the same Aide had observed a resident in his room. “He made his bed and was already dressed up.” Two minutes later, when the Licensed Vocational Nurse returned to the resident’s room, the resident was missing. By 5:02 AM, the nursing staff notified the police of a missing resident.
