The state investigators determined the nursing home "failed to prevent a dependent resident from falling out of bed during incontinence care.” The resident “fell from the right side of the bed while the bed was in a high position, sustaining a bilateral femoral fracture requiring surgical intervention.”
The investigation involved a cognitively intact resident who was “non-ambulatory.” The resident requires “extensive assistance of two staff for bed mobility and extensive assistance of one staff for toileting and personal hygiene.” The resident has “one-sided impairment on the upper extremity and bilateral impairment to the lower extremities.”
At 6:45 AM on October 8, 2018, the resident “was heard screaming.” A nurse entered the room and observed the resident “lying on the floor. She complained of pain to both knees.”
A Nursing Assistant stated that she rolled the resident to change her, and the resident “was trying to get something off the nightstand.” At that time, the resident “rolled onto the floor.” The resident “requested to be transferred to the hospital for an evaluation.”
The “facility failed to provide supervision to prevent the elopement, [wandering away] from the facility, of [a] moderately cognitively impaired resident reviewed for accidents.”
The investigation revealed that the resident was unsupervised by staff when they “exited the facility by a wheelchair and traveled approximately 440 feet (146 yards) to a convenience store behind the facility on a sidewalk.” Law enforcement located the resident who “was returned to the facility with no injuries after an undetermined amount of time outside the facility.”
