The Grand Rehabilitation and Nursing at Mohawk
In addition to providing around the clock skilled nursing care, The Grand Rehabilitation and Nursing at Mohawk offers other services. Additional focused care includes Alzheimer’s/memory care, IV (intravenous) therapy, dental services, bariatric care, oxygen therapy, enteral therapy, ambulation assistance, and restorative services involving physical, speech and occupational therapies.
The investigators working for the state of New York and the federal government had the legal authority to impose monetary fines and deny payment for Medicare services if the nursing home has been cited for serious violations of established regulations and rules.
This nursing facility received two complaints and self-reported two serious issues over the last three years that resulted in violation citations. Additional documentation about fines and penalties can be found on the New York State Nursing Home Report Website.

According to Medicare, this facility maintains an overall rating of one out of five stars, including one out of five stars concerning health inspections, two out of five stars for staffing issues and two out of five stars for quality measures.
The state surveyors determined that “the facility did not ensure all alleged violations involving mistreatment, neglect, or abuse were investigated or reported to the New York State Department of Health as required. Specifically, [one resident] had a skin tear of unknown origin and an investigation to rule out abuse, neglect, or mistreatment was not initiated.”
Reviewing the resident’s Comprehensive Care Plan dated February 26, 2018, explained that “the resident was at risk for skin tears related to fragility and impaired cognition. Interventions included Dura-sleeves on at all times, [to be] removed in the morning and at bedtime for hygiene care.” However, an updated Note in the Comprehensive Care Plan documented that “the resident had no current skin tears and continue to remain at high risk.”
The surveyors observed the resident in the early afternoon of March 13, 2018, “with a gauze bandage wrapped around her left forearm, secured with medical tape, and no date was marked on the tape.” A family member told the surveyors at that time that “the resident had very fragile skin, had multiple skin tears in the past, and she was not notified of any recent skin tears.” The family member stated that “she did not know the reason for the bandage on the resident’s left forearm.”
Two days later, during the lunch, “the resident was observed in the dining room. She and undated gauze bandage wrapped around her left forearm. The resident’s family member stated she inquired about the bandage and was told it was a skin tear. She was not advised of when or how it occurred.”
The surveyors reviewed all the Nursing Progress Notes for the first two weeks in the month of March. However, “there was no documentation found regarding a skin tear to the residents left forearm, and assessment, or treatment order for the area.” There was “also no documented evidence of an incident report initiated related to the resident skin tear to the left forearm.”
