The state investigators determined that the nursing home “failed to ensure staff treated one cognitively impaired resident with respect and dignity one license nursing staff locked the brakes of the resident’s wheelchair remove the resident’s nonslip slippers and replaced them with regular black slacks to slow the wandering resident down and where the resident out.”
A review of the resident’s Annual MDS (Minimum Data Set) Assessment revealed the resident had short and long-term memory problems, and “physical behaviors directed toward others, verbal behavior is directed toward others, and other behaviors not directed toward others.”
The documentation shows that “the resident used a walker and wheelchair for mobility and requires extensive assistance of one staff with walking in the room, limited assistance of one staff was walking in the corridor and required supervision and set up help only with locomotion on-and-off the unit.”
In a separate summary statement of deficiencies dated October 10, 2018, the nursing home “failed to identify patterns of abuse exhibited by [a resident] directed toward cognitively impaired residents with wandering behaviors. On August 20 a 2017, “the resident] identified as alert and oriented, slapped a cognitively impaired resident.” On September 1, 2018, the same aggressive resident slapped a third cognitively impaired resident on the hand.
It was determined that “this deficient practice placed [both assaulted resident’s] in immediate Jeopardy.”
