  • Failure to Provide and Implement an Infection Protection and Control Program – citation #F880 date November 8, 2018
  • The state investigators determined that the nursing home “failed to meet infection control standards of practice as recommended by the CDC (Centers for Disease Control and Prevention) to prevent an outbreak of scabies.” The investigation involved four residents on isolation precautions.

    The state investigators observed “six carts containing personal protective equipment (PPE) on the 200 Hall. In one case, a pink sign was affixed to [one resident’s] door that indicated to use caution when entering the room and to wear gloves, a mask, a gown, and goggles.” However, there were “no check marks giving any indication of what type of PPE was needed for the care of [this resident].”

    The investigators noted that “there were five other locations on the 200 Unit with isolation carts, three of them had signs on the corresponding door without any indication of what type of PPE was indicated for that specific isolation, and two carts had no signs on the corresponding door.”

    The 400 Hall Nurse revealed that “it is everyone’s responsibility to stock the isolation cart when they run out of supplies.” The Assistant Director of Nursing stated that “signage and carts for isolation rooms are his responsibility to see that they’re stocked."

    The investigators asked if the Assistant Director “was aware of the type of isolation for [two residents sharing a room].” The Director stated that “he was aware. When he was told that there was no signage for the type of isolation or what type of PPE should be worn on the doors for the room with [those two residents] as well as two other resident rooms and that most of the carts contained only gloves and gowns, he replied that he would look into it.”

    A Certified Nursing Assistant revealed that “she had never seen hair coverings or booties in any isolation carts, and she added ‘I’ve been working here for many, many years.’”
