The state investigators determined that the nursing home “failed to provide adequate supervision and thoroughly investigate an unwitnessed fall for [one resident who] sustained two unwitnessed falls on May 9, 2019, and May 17, 2019.” It was determined that “not providing adequate supervision and a failure to thoroughly investigate an unwitnessed fall placed the resident at a higher risk for injury and additional falls.”
The state investigators reviewed the resident’s MDS (Minimum Data Set) Assessment showing the resident was “non-ambulatory, requires extensive assistance of one person for bed mobility, and extensive assistance of two persons for transfers.” The resident’s medical records revealed that the resident “had two documented unwitnessed falls” over eight days.
The facility Incident & Accident Form involving the first incident revealed that the resident “was heard yelling for help and found lying on the floor in his room. He had no injuries.” Documentation revealed that the resident “told the staff he was trying to get out of bed when he fell.” The staff implemented interventions, including ensuring that the bed is placed in the lowest position and installing bilateral floor mats.
A subsequent Incident & Accident Form involving the second fall revealed that a Certified Nursing Assistant (CNA) “returned from a lunch break and found the resident on the floor next to his bed.” A Licensed Vocational Nurse went with the CNA into the resident’s room after an unwitnessed fall.
The resident’s Plan of Care was updated to include goals to “minimize the risk for falls in the next three months and weekly significant injury as a result of the falls in the next three months.” However, “there were no new interventions documented on the Plan of Care to show how the facility was going to prevent additional falls to meet the above-identified goals.”
The Licensed Vocational Nurse reviewed the resident’s Plan of Care and verified “none of the Care Plan problems included new interventions after the second fall or to keep the resident’s bed in the lowest position.”
