According to Medicare, this facility maintains an overall rating of two out of five stars, including one out of five stars concerning health inspections, four out of five stars for staffing issues, and four out of five stars for quality measures.

  • Failure to Ensure Services Provided by the Nursing Facility Met Professional Standards of Quality – citation #F658 date October 5, 2018
  • The state investigators determined that the nursing home “failed to ensure that Neurological Evaluations were completed per facility protocol after an unwitnessed fall for [one resident] reviewed for falls.” The investigation involved a resident who “sustained an unwitnessed fall on August 28, 2018.”

    A review of the resident’s Nurse’s Notes from the same day revealed that the resident “fell while trying to stabilize himself on a shower chair.” The resident denied hitting his head but complained of hip pain. The nursing staff ordered an x-ray. Neurological checks begin.

    The facility notes indicate that the Physician “did examine [the resident] shortly after the fall, which included a neurological evaluation.” However, reviewing the resident’s records between August 28, 2018, and August 30, 2018, revealed “no Neurological Evaluation Flow Sheet and only two Nurse Progress Notes.”

    One of those notes stated that the resident was in bed and no longer complained of pain. The document shows that there were neurological checks every two hours per the facility’s protocol. Later that day, another report indicated that the resident “has good rehabilitation potential and complies with the medication and treatment regimen.” The document says that there is a “fall precaution infection control program promoted at all times.”

    However, by August 30, 2018, at 9:45 PM, the Change of Condition Form indicated a change in acute mental status related to “[the resident] seeing people in the ceiling and coming down the wall. [The resident] was placed on one-on-one observation and sent to the hospital due to altered mental status.”

    The Director of Nursing stated on October 4, 2018, that “there were no Neurological Evaluation Flow Sheets found [for the resident].” The Risk Manager stated that “there is a form that they are to use to document this.” The state investigation included a review of the facility’s policies, staff interviews, and record reviews.
