According to Medicare, this facility maintains an overall rating of one out of five stars, including one out of five stars concerning health inspections, two out of five stars for staffing issues, and four out of five stars for quality measures.

  • Failure to Ensure That Every Resident Is Free from the Use of Physical Restraints Unless Need for Medical Treatment – citation #F604 date January 2, 2018
  • The state investigators determined that the nursing home "failed to ensure continuous appropriate assessments of less restricting measures [before] using physical restraints.” The nursing home also failed to “ensure a resident was free from physical restraints by placing bed linen around the resident’s waist without a Physician’s order.”

    The state investigators determined that “these deficient practices have the potential [to cause a] negative psychosocial impact for [one resident’s] well-being by being restrained.” The investigators reviewed a Licensed Nurse’s Progress Note documented at 4:00 AM that indicated that the resident “was trying to get out of bed, the nursing staff tried to help her to avoid a fall and injuries from a fall.” The resident “was combative and aggressive to the staff. She started to throw herself on the floor with her uncontrollable body.”

    The document indicates that the “Charge Nurse instructed a Certified Nursing Assistant (CNA) to apply linen around the resident’s waist and put her in her wheelchair to monitor closely.” A review of the resident’s Physician’s orders indicated that “prior to using physical or chemical restraints, the facility should use less restricting measures that may use a tab alarm/padding alarm while in a wheelchair or bed to anticipate immediate needs.”

    There was a New Physician’s order that indicated that “the resident may have a self-release belt for safety for the resident’s uncontrollable behavior by throwing herself on the floor, aggressive/combative behavior. If behaviors continue, request one-on-one [supervision].” The document also indicates that the nursing staff was to discontinue a lap buddy order. However, “there was no evidence of a written order to apply a lap buddy [before] the discontinuation order.”

    The Director of Nursing indicated that the resident had behavioral issues, was very agitated and aggressive toward others and stated that “the staff member restrained the resident to protect her from a fall.”
