According to Medicare, this facility maintains an overall rating of one out of five stars, including one out of five stars concerning health inspections, two out of five stars for staffing issues and three out of five stars for quality measures.

  • Failure to Report and Investigate Any Act or Reports of Abuse, Neglect or Mistreatment of Residents – citation #F225 date July 29, 2016
  • According to state surveyors, “the facility did not ensure that all alleged violations involving potential misappropriation of resident property were thoroughly investigated.” The incident involved two residents “reviewed for complaints and grievances. Specifically, [for one resident] the facility did not complete an investigation after the resident reported that sixty dollars was missing.”

    Also, “the facility did not complete an investigation after [the other resident] alleged a staff member took her vase.” The surveyors reviewed a Concern/Complement Form that documented the resident “notified the Social Worker that when she returned from the hospital, sixty dollars was missing. When she left for the hospital, the money was in her nightstand.”

    A second Concern/Complement Form documented that the resident “notified the Social Worker that her vase that she kept near the door was missing and named a specific staff member whom she believed took the vase. The Social Worker documented that they were “unable to locate the vase, and the staff would continue to look for it. There was no statement attached to the form.”

  • Failure to Ensure the Nursing Home Area Remained Free of Accident Hazards and Risks and Provides Supervision to Prevent Avoidable Accidents – citation #F323 date July 29, 2016
  • The nursing home “did not ensure that each resident received adequate supervision to prevent accidents.” Specifically, for one resident, “the facility did not implement interventions, including adequate supervision, consistent with the resident’s need [to] reduce the risk of an accident occurring.”

    On January 7, 2016, at 10:00 PM, the resident “struck her roommate with a broken towel bar.” Just after midnight, the resident “was moved to another unit.” The aggressive resident “continued to ambulate into other residents’ room without supervision.”

    On July 20, 2016, the resident was “observed independently ambulating on the unit. She was observed placing her hands in the corridor clean linens cart in the hallway. She was observed entering another resident’s room for approximately two minutes, left the room, and ambulated toward the nursing station. She was then observed entering Room 241 and was overheard acknowledging [the aggressive resident] by name.”

    The aggressive resident was seen entering and exiting other residents’ rooms during the observation.
