  • Failure to Provide Every Resident an Environment Free of Accident Hazards and Provide Adequate Supervision to Prevent Avoidable Accidents –date February 22, 2019
  • The state investigators determined that the nursing home "failed to ensure the use of a smoking apron during smoking for [one resident]. This failure increased the risk for [the resident] to sustain burns.”

    The survey team reviewed the resident’s clinical records and Physician’s orders for drugs to treat tremors. During two separate observations over two days, the resident “was observed smoking on the patio without a smoking apron and without staff supervision.” The resident “was observed with bilateral hand tremors.”

    The resident stated that “she lights her own cigarettes [and] she smokes one or two cigarettes before and after breakfast, lunch, and dinner.” The resident “denied she was supervised by staff while she smoked saying, ‘the nurse makes me a cup of coffee. I have tremors and it’s gotten bad. I cannot do it (make a cup of coffee) myself anymore.’”

    A Licensed Practical Nurse (LPN) providing the resident care stated that the resident “wore her smoking apron 60% of the time while she smoked [saying that the resident] required a smoking apron because of her tremors.” A review of the resident’s Care Plan indicates that the “resident is an active smoker [and] smoked with an apron on whenever she was going out to smoke, as she has bilateral hand tremors.”

    The Director of Nursing confirmed that the resident “did not wear an apron every time she smoked.”

  • Failure to Attempt to Different Approaches before Using a Bed Rail – citation #F700 date February 22, 2019
  • The facility “failed to ensure side rail assessments for entrapment (a situation from which it is difficult or impossible to escape) risks were completed [before] resident use.” This failure “had the potential to cause a restricted exit from the bed, increased risk of injury, and lead to decline in functional status (an individual’s ability to perform normal daily activities required to meet basic needs, fulfill usual roles, and maintain health and well-being).”

    A Licensed Practical Nurse (LPN) stated that this resident was not assessed for entrapment risks due to side rail use. Both the Administrator and Director of Environmental Services confirmed that “the facility did not have a process in place to assess for entrapment risks.”
