According to Medicare, this facility maintains an overall rating of two out of five stars, including two out of five stars concerning health inspections, three out of five stars for staffing issues and three out of five stars for quality measures.

  • Failure to Provide a Safe, Appropriate Pain Management Plan of Care for a Resident Who Requires Those Services – citation #F697 date December 21, 2017
  • The state investigators determined that the nursing home failed to ensure the pain management was provided to one resident by “allowing the resident to go two days without having Physician-ordered medication available to treat the resident’s pain.”

    The state investigators interviewed the resident who stated that “the facility had let her run out of pain medication on Friday, December 15, 2017, and that she had gone all weekend without her pain medication. She stated that she was at the facility after fracturing her hip and undergoing corrective surgery.”

    The resident also stated that she had “constant pain in her back and stated that she was not able to take over-the-counter medications such as Tylenol because she had a liver condition.” The resident said that “staff was aware and told her that when the count of her medication got low, the pharmacy was supposed to send more, but could not explain to the resident why this had not happened in her case.”

    The resident stated that “she was miserable at times through the weekend.” The record review for the resident showed that “she was admitted to the facility after suffering a fall that resulted in surgical intervention to repair a nondisplaced femoral neck fracture.”

    During a follow-up interview, the resident stated that “the nurse on duty over the weekend had matter-of-fact [attitude] that she was out of pain medication and told her she would not be able to get her anything but Tylenol until the following Monday, December 18, 2017.”

    The resident stated that “the weekend nurse never mentioned an emergency box of medication, but that on Monday when she complained to the Unit Manager, she was given medication at 3:30 PM from the emergency box because the normal shipment did not come until 4:00 PM.”
