According to Medicare, this facility maintains an overall rating of two out of five stars, including two out of five stars concerning health inspections, two out of five stars for staffing issues and five out of five stars for quality measures.

The state investigators determined that the nursing home “failed to ensure floor cleaning equipment was in proper working condition to prevent falls for [one resident].” The deficient practice by the nursing home employees resulted in the resident “sustaining a right wrist fracture.”
A Nurse’s Note indicated that the resident sustained “an unwitnessed fall.” The conclusion of the final investigation identified the cause of injury “to be a slip and fall due to residual water from the housekeeping machine.”
At the time of the incident, the housekeeper “immediately cleaned up the residual water from the floor and notified [the Housekeeping Supervisor].” The supervisor determined, “there was a malfunctioning hose on the machine and immediately notified [the Maintenance Supervisor] who then repaired the machine.”
The injured resident received an x-ray of the wrist and orthopedic consultation. The orthopedist ordered a “short arm cast [that] is currently in place.” The resident’s Orthopedic History and Physical documents “a mildly displaced dorsal triquetrum fracture with soft tissue swelling.” The resident was to receive Tylenol pain medication while healing before the two-week follow-up.”
The resident stated that “there was water on the floor, and I fell.” A Unit Aide stated, “I saw [the resident] come around the corner and [the resident’s] sandal hit the water just right and [the resident] fell.”
The nursing home “failed to appoint a Registered Nurse as the Director of Nursing. This failure has the potential to affect all ninety-eight residents residing at the facility.” The facility assessment documents indicate that the nursing home “is licensed for 123 beds and they are predominantly long-stay residents.”
At the time of the survey, the Director of Nursing position was vacant, and there had not been an active Director of Nursing since April 10, 2019, more than three months earlier.
