According to Medicare, this facility maintains an overall rating of two out of five stars, including three out of five stars concerning health inspections, three out of five stars for staffing issues and one out of five stars for quality measures.

  • Failure to Ensure That Every Resident Is Free from the Use of Physical Restraints Unless Need for Medical Treatment – citation #F604 date July 19, 2018
  • The state investigators determined that the nursing home “failed to ensure [one resident] had been assessed for the need of physical restraint.” The resident “was observed in her wheelchair with a padded laptop device in place” on three different occasions between July 17, 2018, and July 18, 2018.” At 2:00 PM the second day, the resident “stated she was not able to remove the padded lap device.”

    The Director of Nursing stated that “the facility did not have a restraint assessment for [that resident] and he believed therapy had placed the padded laptop device” in the resident’s wheelchair in the months previous.

    The Physical Therapy Assistant stated that the previous Director of Nursing had “applied the padded lap device for [the resident] before therapy treated her.” The Assistant stated that the resident “could be assessed for the use of the padded lap device if therapy was given a doctor’s order for that assessment.”

    The Physical Therapy Daily Treatment Note dated February 22, 2018, documents that “the padded lap device was applied per the Director of Nursing for keeping the patient in the wheelchair safely with a history of sliding from the wheelchair to the floor.”

  • Failure to Provide Appropriate Pressure Ulcer Care Prevent New Ulcers from Developing – citation #F686 date February 28, 2019
  • The facility “failed to provide effective clinical infection control practices during wound care treatment for [one resident].” The investigation involved a resident observed receiving wound care to the buttocks.

    The Registered Nurse providing the resident care acknowledged that the resident “is on Contact Isolation Precautions” due to a highly contagious disease. The surveyors observed the RN cleansing the resident’s buttocks using wound cleanser, removing her gloves, washing her hands, then opening the isolation trash bin with ungloved hands.”

    The Director of Nursing confirmed that she would not expect “staff to touch the isolation bins with bare hands after performing hand hygiene” and not expect “staff would reapply a soiled incontinence brief [as was done] after providing wound care.”
