She also stated she was scared of [her husband].”

    A nursing staff member said that “she had worked one-on-one with [the husband] when he hit [his wife] on the left shoulder on December 10, 2018.” The staff member stated that “she did not report the allegation of abuse to the nurse or Administrator.” The facility Administrator confirmed that “he spoke with the nursing staff regarding the allegation of abuse for [the married couple]. He stated the [nursing staff member] should have reported the incident immediately to the nurse or Administrator” as required by policy, procedure, and law.

  • Failure to Provide Every Resident an Environment Free of Accident Hazards and Provide Adequate Supervision to Prevent Avoidable Accidents – citation #F689 date August 21, 2019
  • The “facility failed to ensure a [Certified Nursing Assistant] went out to look for [the resident] when the receptionist informed her [the resident] was outside on the street on August 10, 2019.” The CNA “did not look for the resident in a timely manner, which resulted in police finding [the resident] six days later in a place which was about 80 miles away from the facility.”

    The investigators determined that “this failure had the potential for [the resident] to have an accident or injury while out of the facility.”
