The state investigators determined the nursing home "failed to follow procedures to report the allegation of abuse for [one resident].The nursing staff did not report the allegation of abuse when [one resident hit another resident] on the left shoulder. This failure placed residents in potential harm of further abuse.”
The resident was interviewed while “sitting in her bed and crying.” The resident stated that another resident “who was her husband suddenly hit her on the left shoulder.” The reporting resident said that “there was nursing staff present when the incident happened. She also stated she was scared of [her husband].”
A nursing staff member said that “she had worked one-on-one with [the husband] when he hit [his wife] on the left shoulder on December 10, 2018.” The staff member stated that “she did not report the allegation of abuse to the nurse or Administrator.” The facility Administrator confirmed that “he spoke with the nursing staff regarding the allegation of abuse for [the married couple]. He stated the [nursing staff member] should have reported the incident immediately to the nurse or Administrator” as required by policy, procedure, and law.
The “facility failed to ensure a [Certified Nursing Assistant] went out to look for [the resident] when the receptionist informed her [the resident] was outside on the street on August 10, 2019.” The CNA “did not look for the resident in a timely manner, which resulted in police finding [the resident] six days later in a place which was about 80 miles away from the facility.”
The investigators determined that “this failure had the potential for [the resident] to have an accident or injury while out of the facility.”
