The state investigators determined that the nursing home “failed to ensure that it provided adequate supervision and assistance to prevent falls for [one resident] who had unwitnessed falls on May 31, 2017, June 2, 2017, and June 4, 2017.” The three falls occurred at 8:40 PM, 6:20 PM, and 11:00 AM on the respective days.
A review of the resident’s Care Plan and the focus area indicates that the patient “has the potential for injury from falls related to decreased vision (diagnosis) glaucoma and macular degeneration, prefers to wear personal socks, incontinent episodes, kind of deficit, poor safety awareness, and utilizes a wheelchair as ambulation.” The document also indicates that the resident “has a history of falls, a diagnosis of osteoporosis, and poor posture when ambulating.”
The Care Plan also indicates that the resident “needs a safe environment with even floors free from spills or clutter” and a glare-free light environment with the bed in the lowest position at night and handrails on the walls. All personal items are to be kept within reach.
A Progress Note dated May 31, 2017, at 9:20 PM indicated that the resident was “found sitting on [their] buttocks on the patio.” The following day, the interdisciplinary team met and placed interventions to ensure resident safety.
However, two days later, the resident returned from the hospital in a wheelchair transported by her son complaining of no distress or pain. The following day on, June 4, 2016, the resident was being helped to the restroom and was asked to pull the string when she was done to receive further assistance. However, the resident “never asked for help and got back to the bedside on her own.” Minutes later, the resident was “found sitting on the floor by her bed with her legs crossed.”
