According to Medicare, this facility maintains an overall rating of one out of five stars, including one out of five stars concerning health inspections, three out of five stars for staffing issues, and three out of five stars for quality measures.

  • Failure to Provide Every Resident an Environment Free of Accident Hazards and Provide Adequate Supervision to Prevent Avoidable Accidents – citation #F689 date March 26, 2019
  • The state investigators determined the nursing home "failed to implement fall prevention interventions for [one resident] when [the resident] sustained a fall after a Certified Nursing Assistant (CNA) transferred the resident from his bed to a shower gurney alone and without following facility safety procedures for dependent residents who require two-person assistance for transfers.”

    The deficient practice by the nursing staff resulted in the resident’s “ground-level fall [and] emergency room evaluation for a lip laceration.” The CNA involved in the incident stated that the resident was about to receive a shower. She attempted to transfer the resident “from his bed to the shower gurney alone and with the use of a transfer board.” The CNA stated the resident “fell on top of her after she turned him toward her on the transfer board.”

    The CNA stated that “she lost her footing and [the resident] fell on top of her.” A Registered Nurse (RN) recalled the incident involving the fall and stated that the CNA never asked for help from any other staff member. The Director of Nursing said, “we use two-person assist for transfers with total dependent residents.”

    In a separate summary statement of deficiencies dated March 21, 2019, the facility “failed to maintain a hazard-free environment for [one resident] when the facility knew [the resident] did not have the mental capacity to make his own decisions and exhibited exit-seeking behaviors.”

    Also, the facility “did not assess or implement interventions to prevent him from leaving the facility and allowed him to sign himself out of the facility against medical advice (AMA) (the decision to discontinue Medicare or treatment services despite the advice of medical professionals) on November 19, 2018.”

    The deficient practice by the nursing staff “had the potential to expose [the resident] to harm, injury, or death.” The Director of Staff Development stated that “according to the clinical record, [the resident] was originally found wandering in traffic and taken to an acute hospital.” The Director also verified that the resident “did not have the capacity to make decisions.”
