  • The state investigators determined that the nursing home “failed to supervise residents at increased risk for falls, failed to avoid resident falls, and failed to revise interventions to prevent further falls when necessary.”

    As a result of these failures, one resident “was transferred to the hospital after an unwitnessed fall.” The resident “returned from the hospital three hours later” with the diagnosis of injuries. A second resident also fell in a separate incident.

    The second (severely, cognitively impaired) resident “did not have a history of falls” but was scored “at risk for falls and recommended an initiation of a fall risk intervention.” The individual needs extensive assistance of two or more individuals and is non-ambulatory.

    At approximately 3:00 PM, the resident was found on the floor with pain to the left hip. This resident was “admitted to the local hospital for surgical repair of the left hip after falling in [their] room.”

    At the time of the incident, the nursing staff failed to initiate a fall Care Plan that was only started after “[the resident] fell and fractured [their] femur. The fall prevention interventions list included encouraging [the resident] to wear non-skid footwear when ambulating or mobilizing a wheelchair [and] encouraging [the resident] to use the call light.” However, the resident “is confused and unable to follow staff instructions on the use of call lights and is unable to understand being encouraged to participate in activities.”

    The Director of Nursing admitted “having a resident who had falls with fractures and later provided a list of residents with falls.” The Director stated that “Care Plans are to be updated after each fall as are the Fall Risk Assessments.”
