  • The state investigators determined that the nursing home “failed to ensure the policy for resident abuse was followed for [two residents].” Specifically, the “facility did not do a complete investigation or have complete documentation regarding abuse allegations.”

    The state investigators interviewed the Nursing Home Administrator who stated that a Department of Family and Children Services (DCF) worker “came into the facility [and] asked for the admission assessment, face sheet, Medication Administration Record and Treatment Administration Record for [a resident].”

    The administrator stated that “the floor nurse explained to her about her area (on the buttocks) and it was due to shearing.” The administrator stated that “[the resident] had a treatment order and sometimes [the resident] would refuse to wear a brief.” The Administrator also indicated that the resident suffered from “bouts of diarrhea.”

    The administrator stated that “to her knowledge, [the resident’s] treatments were being taken care of.” However, both the Administrator and Director of Nursing stated that “no assessment was performed by the Director of Nursing and [that] no internal incident report was done.” They stated that “there were no statements from people possibly involved.”

    The Nursing Home Administrator said that the DCF worker “did not say abuse, she said care concerns. The Director of Nursing stated that she did not recall the DCF worker speaking with her regarding [the resident].” Both staff members stated that they “were not aware of what DCF’s concern was.”

    The Administrator was asked about grievances that have been filed by the resident. The Administrator stated that “[the resident] was mistaken on what he thought she said. The Nursing Home Administrator went on to say that the Certified Nursing Assistant (CNA) was educated regarding customer service.”

    The Administrator indicated that the resident “had made sexist comments to staff members as well as his wife and was verbally aggressive.” The surveyors asked the nursing home administrator “if there was any documentation of [the resident] displaying inappropriate and aggressive behaviors and she stated that there was not.”
