The state investigators determined that the nursing home “failed to implement a resident’s Plan of Care, which included visual checks and a functioning bed alarm.” The resident “was assessed as being at a high risk for falls.” The deficient practice by the nursing staff resulted in a severely cognitively impaired resident “falling from the bed and sustaining a 1.05 cm deep” laceration, which resulted in “a transfer to the General Acute Care Hospital for head trauma and laceration repair.”
A review of the resident’s medical records indicated that the resident had been receiving antipsychotic medications and that Certified Nursing Assistants (CNAs) “were to provide adaptive devices and observe [the resident]. The Care Plan indicated that all staff were to apply the resident’s bed alarm while the resident was in bed and the wheelchair in place and a commode by the bedside.”
The resident had verbalized they had a fall on July 5, 2017. The “Care Plan did not indicate a set time, frequency, and duration of when the nursing staff was to perform monitoring of [the resident].”
The Emergency/Urgent Care Notes from the hospital indicate that “the resident had an unwitnessed fall from a commode with a scalp laceration requiring three sutures.” The Progress Note indicates the resident “had a 1-inch laceration with three staples [and] skin discoloration on the left lateral knee, redness to the back of the base of the neck and left lateral back.”
The state investigators observed the resident in bed. The bed was “in the low position with no floor mats on either side of the bed, and no visible bed alarm.” The investigators observed the resident’s family members “gathered around the resident crying in tears.” The resident “was non-responsive to verbal commands and directions from family members.”
A Licensed Vocational Nurse stated that the resident “had fall preventative measures, such as a bed alarm to sound off whenever [the resident] would try to get up from the bed.” The nurse stated that “she did not hear any alarms going off at the time of [the resident’s] fall on May 17, 2017.”
