The state investigators determined that the nursing home “failed to provide care and services for [one resident] with diabetic care to administer insulin per Physician’s orders.” The nursing home also “failed to provide appropriate diabetic care during a hypoglycemic episode per the standard of care for [a different resident].”
The survey teams interviewed the resident who indicated that the nurses at the facility, or approximately half of them, “go give me my insulin before lunch like they are supposed to. I will tell them that they did not give it to me, and they will tell me they did.”
The cognitively intact resident indicated that the nursing staff “made me feel like I am losing my mind. I have some health problems, but I am not crazy.” The survey team then made observations of the resident on May 26, 2017, between 10:40 AM and 12:10 PM. During that time, the resident was in his room, [taken] to the therapy department, then back to his room, and then out to the dining room.
The nursing staff provided the resident juice and coffee before being served “his meal of a tuna sandwich, cucumber salad, and dessert. The nurse did not check the resident’s glucose levels before [the resident] ate lunch.”
Eight minutes later, the resident was interviewed again while “sitting in the dining room [completing] his lunch. [The resident] was asked, ‘did the nurse give him his insulin.’” The resident replied, “no, she did not.”
The surveyors interviewed a Licensed Practical Nurse (LPN) and the Director of Nursing a few minutes later and asked the LPN “did she check the blood sugar and administer insulin to [the resident] before lunch?” The LPN replied that “No, I did not.”
When the LPN was asked, “why would she document that she had administered insulin when she had not?” The LPN replied, “I do not know. I guess it was because I did not know where he was.”
