The state investigators determined that the nursing home “failed to provide supervision to [a resident] and failed to implement the facility policy and procedure to assess and provide care and treatment after a fall. The facility also failed to ensure fall prevention interventions were implemented and revised after multiple falls for [another resident].”
It was determined that the failure by the nursing staff resulted in the first resident sustaining serious injuries of the right ankle “that the facility was not made aware of.” This resident was observed “sitting in her wheelchair with a short leg cast on her right foot.”
At that time, the resident stated, “I fell a few weeks ago, and I broke my ankle. I think I tripped on something or sometimes when I get up at night; I get dizzy. I have been falling a lot lately. They (the staff) do not really check on me at night even if you call for assistance. If they ever come, sometimes I have already forgotten what I needed from them.”
During an interview with a Licensed Practical Nurse (LPN), it was explained that “I went to [the resident’s] room, she told me that she fell and her ankle hurts. Her right ankle was swollen. I called the doctor, and I recommended to have x-rays be done. No, I did not request for a stat [immediate] x-ray. My shift ended at 6:00 AM, so I left at 6:00 AM. I do not know when they did it (the x-ray).”
It was determined that the nursing staff failed to document a head to toe assessment performed or identify any limits to the range of motion. The LPN involved in the incident when it was first identified stated: “No, I did not check for range of motion.” Another nurse indicated that the patient has an unwitnessed fall; the “nurse should assess the patient from head to toe to identify the injury sustained and the range of motion should also be done.”
