The state investigators determined that the nursing home “failed to administer pain medications [timely] for pain. The failure caused [the resident] to be in severe pain for thirty minutes.” Additionally, “the facility failed to follow fluid instructions for [another resident].”
The state investigators interviewed the resident, who stated that the facility “did not provide pain medications to him [timely].” The resident “reported that he was in pain at that time. He stated he received the last pain medication at approximately 12:00 PM” approximately three hours earlier.
The resident indicated that his pain medication was prescribed to be given as needed, every two hours, and as needed when asked for. The state surveyors observed the resident pressing the call light button and telling the Aide at 3:11 PM “that he was in pain and needed pain medication.”
The Aide stated that she would inform the nurse and returned approximately a minute later to tell the resident that there was a switch of staff members and that the oncoming staff members would provide the resident care in approximately ten minutes. The surveyors watched the evening shift nurse at 3:26 PM by the resident’s room and visiting other residents’ rooms with the medication cart.
The nurse was observed going into another resident’s room at 3:34 PM, and a third resident’s room at 3:38 PM. By 3:40 PM, the nurse entered the resident’s room. One minute earlier, the resident stated to the surveyor that “he had not received the requested pain medication [and that now] his pain was at eleven and confirmed that he was using a scale of 1 to 10 with 0 indicating no pain and ten indicating the most pain.”
Three minutes later, the surveyors interviewed the nurse in the presence of the aide who indicated they would receive their pain medication in ten minutes. The nurse stated that “she forgot about [the resident’s] pain medication.”
The facility Director of Nursing reported that “she was unaware of the staff’s failure to administer the pain medication but would investigate it.”
