According to state investigators, “the facility failed to ensure a resident was free from unnecessary medications without indication for use.” The surveyors observed the resident asleep in bed at 1:26 PM on March 2, 2018. Forty-one minutes later at 2:07 PM, the resident “was walking to the nurse’s station phone to take a call from a family member.”
Three days later, on March 5, 2018, at 8:16 AM, the resident “was in bed with her eyes closed.” Two days later, on March 7, 2018, at 11:04 AM, “she was in bed asleep.” The surveyors reviewed the resident’s Current Medication Order that included a nicotine patch 14 mg/24 hours to be applied once daily with the old patch removed.”
The nursing staff had been administering the Physician’s order nicotine patches since the resident had been admitted to the facility. However, when the surveyors interviewed two Certified Nursing Assistants (CNAs) who indicated that “the resident did not smoke and had never mentioned wanting to.” One CNA indicated that “she probably would not remember if she did smoke.”
In Licensed Practical Nurse (LPN) indicated that “the resident had never asked her what the patch was for when she applied daily.” The Director of Nursing stated that the resident “had smoked at home [before] admission to the facility [but claimed that] the nicotine patch had not been addressed since it was started.”
The nursing home “failed to ensure completion of routine vaccinations for [four residents] reviewed for influenza and pneumococcal vaccinations.” Facility documentation revealed that for residents who had previously received the pneumococcal vaccination in previous years had not received the vaccine this year.
The facility “failed to ensure infection control practices were followed while passing ice to residents for one of two halls observed and during two of six dining room observations.” The staff failed to follow handwashing technique and preparation of equipment in the facility’s dementia unit to prevent the possible spread of infection.
