Missouri state and federal regulators have the ability to impose a fine on nursing home operators when there are certain serious health or safety violations that are noted on an inspection of the facility or when the facility fails to take corrective action in a timely manner after violations have been noted. Ackert Park Skilled Nursing & Rehabilitation Center was assessed a $42,565 fine in December 2016. In addition, this facility was denied payment by Medicare for 43 days in response to a pattern of serious violations revealed during the prior inspection. Further, a recent program has given Medicare the ability the either give a bonus to or impose a penalty on nursing home operators based on the number of residents who required hospitalization. Ackert Park Skilled Nursing & Rehabilitation Center received a penalty in the period that ended in September 2018. The percentage of its payments that it was penalized was among the largest in Missouri, losing close to the maximum two percent that the program permitted Medicare to impose.
Ackert Park Skilled Nursing & Rehabilitation Center is a one-star rated nursing facility according to the Medicare ratings. This facility received the lowest scores possible for health inspections, staffing and quality of resident care. In addition, the facility has also received ten fire safety citations, which is well above the average in Missouri.
In December 2016, when this facility was assessed its fine, it received 21 separate health deficiencies and it was cited for two instances of mistreatment. After the December visit, a revisit was conducted in February 2017. The facility was cited for failure to contact the Missouri Department of Health and Senior Services within 24 hours after a resident suffered a significant injury of unknown origin. The facility also failed to administer medications to three residents and did not provide proper perineal care for dependent residents. Staff members were also found to not properly wash their hands throughout the day. Prior to this December 2016 fine, there were 11 deficiencies found during the 2015 inspection.
Its December 2017 inspection revealed that the services provided by the nursing home did not meet professional standards of quality. Specifically, the facility failed to follow orders for oxygen administration for a patient in accordance with their treatment plan. Another patient required a C-collar, but the treatment plan did not document the need for this device and the patient was spotted several times around the facility without it. There were also numerous food safety violations and an inadequate program to prevent against infections throughout the facility.
In all, there have been 11 citations issued to this nursing home resulting from patient-filed complaints. Additionally, five more citations have been assessed for violations that were reported by the facility. It total, the Ackert Park Skilled Nursing & Rehabilitation Center has been assessed 42 deficiencies in the three-year period between 2015 to 2017.
