The state investigators determined the nursing home "failed to prevent a dependent resident from falling from the bed and sustaining injuries, during the provision of care.” The investigation involved one resident “reviewed for supervision to prevent accidents.”
A staff member providing the resident with incontinence care was involved “when the resident rolled out of bed and fell to the floor.” The resident “received treatment at the hospital for injuries sustained as a result of this fall.” These injuries included “a laceration to the head that required three sutures and a fractured clavicle.”
The Incident Report, dated May 3, 2019, at 6:14 AM indicated that a Nursing Assistant had reported to a nurse that the resident “was on the floor. He fell to the floor while the Nursing Assistant was turning him, to provide care.”
The Nurse observed a resident lying “in the supine position on the bedside mat,” on the floor. There “was blood noted under his head, along with a cross necklace. Swelling was noted to the left frontal area above the eyebrow, with bleeding noted.” It was a laceration to the back of the head, with a large amount of blood. Emergency Medical Services (EMS) transported the resident to the hospital.
In a separate summary statement of deficiencies, dated May 19, 2018, the facility “failed to provide adequate supervision and implement interventions to prevent a cognitively impaired resident from attempting to choke an alert and oriented resident.”
The aggressive resident had “a known history of depression and anxiety [and had been] experiencing increased irritability with verbal aggression.”
The staff reported that the aggressive resident “got into a physical altercation with another resident, where he went after that resident and hit him.” The aggressive resident “reported feeling increased irritability, aggravation, and having anxiety.”
The MDS (Minimum Data Set) Nurse stated that “they did not see the resident’s hand around the other resident’s neck.” However, the Nurse saw the aggressive resident’s “hands reaching for [that resident’s] neck.”
